Nominations are invited for the 2004 Evergreen Awards.           Nomination Form

Over the years since 1990, The Evergreen State Society has presented Evergreen Awards to more than 180 individuals and organizations. These awards recognize outstanding contributions to the communities of Washington State.

For 2004, the board of directors seeks nominations of organizations offering programs that particularly demonstrate creativity and a spirit of cooperation. The Society has emphasized different themes over the years, focusing attention on outstanding individuals and organizations for different kinds of accomplishments. In every year, the goal has been to bring greater attention to the accomplishments of nonprofit organizations that serve Washington communities.

A complete nomination includes contact information and specifically addresses four topics:

  1. A description of the public benefit work the organization does.

  2. A discussion of the ways the organization balances commitment to its mission with successful creativity in program activities.
  3. Examples of recent collaborative or cooperative efforts that have been beneficial to the communities the organization serves.
  4. Why this organization deserves greater recognition in 2004.

Nominations are invited from anyone who knows about the nominee, whether currently working with the organization, a supporter, a recipient of its services, or simply an observer.

If the review committee has questions or requires further information, you will be contacted directly.

The personal and organizational information contained in the nomination is confidential. Before the awards are announced, the text of its award citation and other statements concerning each organization selected to receive a 2004 Evergreen Award will be presented to the organization for review.

2004 Evergreen Awards will be presented at a reception following the Nonprofit Leadership Conference April 1, 2004, at the Meydenbauer Center in Bellevue.

Each recipient will receive a crystal disk engraved with the year of the award, a certificate describing the accomplishments being recognized, and a contribution of $200 to be applied toward general operating expenses or for any other purpose.
